Circle of Care Day Wellness Program
One of Hospice's latest programs in the making is the Circle of Care Day Wellness Program. Focused on providing individuals with a life limiting diagnosis time away from home where they can meet new people and do activities they like, and respite for their caregivers. The goal of the program is to provide emotional, social and spiritual support grounded in therapeutic recreation principles and approaches to provide individuals time away from their reality of living with an illness.
Held once a month members have the opportunity to participate in activities such as group discussions, baking, board games, painting, and more. The program is held at the hospice and is led by a PSW and a volunteer with support from other staff if needed.
The Heart of Hastings Hospice accepts referrals from anyone; individuals, family members, friends, visiting nurses, physicians, clergy, or home care workers. Use our contact form or call (613) 473-1880 x107 to speak with a Coordinator about our Community Hospice program.
Referral

Upon receiving a referral, our coordinator will contact you to set up a time to connect with you/your family.
Assessment
​
Our coordinator will meet with you at the Hospice or talk to you over the phone to determine eligibility for the program.
Ongoing Program
​
You will be invited to join the program once a month to participate in activities with other group members
