Speak Up (Advanced Care Planning)
Advance care planning is a process of reflection and communication, a time for you to reflect on your values and wishes, and to let others know your future health and personal care preferences in the event that you become incapable of consenting to or refusing treatment or other care. The Advance Care Planning website has many resources to assist you.
Canadian Virtual Hospice
The Canadian Virtual Hospice provides support and personalized information about palliative and end-of-life care to patients, family members, health care providers, researchers and educators.
Hospice Palliative Care Ontario
Caregiving can be incredibly rewarding, and it can be incredibly isolating and stressful too. The HPCO Caregiver Portals were designed to assist you in finding the support you need, exactly when you need it. You can browse the topics in any order you would like to. A First Nations portal is included as well. Visit the website as many times as you like.
